DLA Claim form

Hi Michael, thanks once again for taking the time and trouble to reply. Unfortunately they didn't answer my questions. I think I perhaps didn't explain properly or maybe you didn't understand what I meant. But thanks for the trouble you went to.
Alls well that ends well though, as it looks like orginaldave understood what I meant and supplied the answer about the forms. Thank you very much originaldave.

hi i'm in proccess of completing dla but have made a mess in places quite bad what should i do ? any advice received with grattitude
Hi margy
Whilst its acceptable to cross out and rewrite your answer or attach an additional sheet on a DLA form, (but not tippex out) if you feel that you should complete another you can download one from the link on this thread under #68131 kindly provided by Dave.
