ESA FORM HOSPITAL OR CLINIC TREATMENT was created by janbjan
Afternoon All
I'm just taking a break from filling in my form and thought I would ask for some advice on what I'm not too sure about. I have regular spinal procedures done as an elective day case would you class that on the form as an in patient or an out patient.I get admitted and receive a discharge paper so I would class it as an in patient even though it's a day case. What do you think?

To be claased as an in-patient you need to spend at least one night in hospital. See here

Sorry I'm being a right pain on this one but filling in this form tends to make you like this. They ask you to tell them about all your hospital visits how many years do they want you to go back at this rate I'll never get this form filled in.I've done so many extra pages already and I haven't even got to the proper questions yet if you know what I mean.

They are primaily interested in any hospital visits during the last year, however, if your have been suffering from your conditions for longer, there may be some advantage in going back further to establish how long you have been affected.
