Migration IB to ESA terminal illness

Contact your local DWP office and explain the situation, you will need a DS1500 form completed by your GP, I assume you already have one for your DLA, this should excuse you completing the ESA50.

When you contact the DWP, be patient as the call centre staff may not be fully aware of the procedures surrounding your situation, if necessary, you can escalate the problem to the Customer Services Manager.
