my friends esa 50 ready to post

Hi freind's form ready to post off, additional pages of a4, supporting letters, have to photocopy everything copy for friend to keep, she has been in hospital over the weekend, so our local cmht having been trying to help her fill it all in, and i downloaded the guides and we used them to help her fill in the questionnaire. JUst looking at the form with all the infomration has been one horrendous stress. To everyone going thorugh this and awaiting the best of luck to you all, i sais to her get the form back, one less hassle. She got her reminder letter today that the questionnaire is due back next week. Will keep you all posted
