It good to finally know what the forms are for, but I still don't understand why they sent me an ESA85 and an ESA85a so I've written to ask them. And they told me that the ESA65 was created for everyone that gets a medical! Also was an ESA85s not mentioned on here at one point -what is that for, please?

Please see this reply from a B&W mod -

It looks like ATOS have to generate an "ADD-ON" to the computer generated ESA85, in all WCA's after the 6/6/11.

It's an ESA85S, Personalised Summary Statement,"PSS" and is written along side the ESA85, and put in front of the DM.
