New ESA50 3/11 form was created by jem69
I've just been sent the new form to fill out. Does anyone know if it's OK to send in exactly the same answers as I did last year as nothing has changed for me? It also doesn't mention being able to attach sheets if you can't fit it all in the boxes. It just says continue on page 18 - but that box isn't big enough either - do I just fill in box on page 18 and say 'see attached sheets'?

If you filled in the form last year, then you need to be aware that the ESA descriptors, and consequently the questions, will have changed, although some remain the same.I've just been sent the new form to fill out. Yes, use as many extra sheets of papaer as you need and firmly attach them to your claim form.
