Obviously the same applies to the Mental Health Guide.
If I was filling out the ESA50 again and on the assumption I could not fit my answers onto the form, then I would put all of my content in a seperate document, referencing the answers back to each question on the ESA50 and each answer to the question.
That way I could be sure that all of my answer was being read and not just the first part.
