Hi,
We do not have a list of the DLA forms used. However in general one would normally ask for a copy of any medical report(s) and also the Decision Maker's 'Statement of Reasons' which should explain how the DM came to make the decision arrived at. Of course one could also ask for any other paperwork used in the process of deciding on your claim, or even if one submits a Subject Access Request under the Data Protection act, any documents held in regards to your DLA claim and used in the decision making process of your claim.
Thanks for your reply Pete, in case others might find this useful, I have sent off a Subject Access Request Form to the DWP which can be downloaded here from the DWP website
