and if I want a copy of my esa 65, statement of reasons document,
which office should I request it from? I've tried three times with my local benefit office, the area office that send me the wrag placement letter, to no avail.

Hi S,

As you will see from the information on the link provided above, "ESA Forms",

ESA65
The ESA65, often referred to as the Decision Maker's Statement of Reasons, is issued if you fail to score sufficient points in the ESA medical(WCA) and are found Fit for Work.

So unless you were not awarded ESA, and found "Fit for Work" it seems that there would not be an ESA65 generated.

Hi S,

Where one is placed in the WRAG, we are finding from members that on their medical reports, it is simply saying that they meet the threshhold for the WRAG (LCW), without going into detail of how many points that they scored, and what for.

In other words they are simply saying that you scored more than the 15 points required.
