dla form oct 2011

HI there where can I find the upto date dla claim download able help me to fill this in for mental health and physical health the one i found the questions are not the same or numbered the same as the new forms with oct 2011 in the corner , it is a renewal form but it looks just like a whole new claim form to me , as i have already been on high rate care and low rate mobility for 3 years after a few appeals , but am trying to fill them in myself this time.

These forms really scare me

I am a fully paid member of benefits and work

With respect to your renewal, a renewal should really be treated as a fresh claim, in that you should not assume that the DWP are aware of your current limitations/disabilities/needs.

In other words, it would be advisable to go into detail regarding all of the above.

Have you tried the guides entitled "The Best Possible Disability Living Allowance Claims For Adults On Physical Health Grounds" and "The Best Possible Disability Living Allowance Claims For Adults On Mental Health Grounds" both dated November 2011, in the DLA resources?

If the CAB employ a mental health adviser and you think that it will assist your claim, ultimately it must be your decision as to whether you wish to avail yourself of this facility?

Hi i have had my dla renewal forms for a while now but called to get some help getting them filled in till the 13th of March may current claim runs out on the 12th of april 2012 , is this too short notice to return them for a renewal
