However I can't contact the CAB again until the 19th so I was wondering about what the address would be for the letter my GP would write?

Its impossible to second guess your GP. Quite often they simply put, To Whom it may concern.

I wouldn't worry too much about the actual addressee, as the CAB will know what to do and are Faxing it for you, in any case.
