I won't bother asking the doctor to fill out the ESA113 form to have a home visit. It is too short notice and like you said I may not get one anyway. I will try and get him to fill out a supporting letter for me, though. If he does this for me do I take it with me to the medical or do something else with it?

If my doctor agrees to do a supporting letter for me, does he also have to send a copy to the DWP?

No, your G.P. does not have to send it to the DWP or ATOS, that is generally down to the patient.
