Help with ESA1 form.

Hi everyone,

I'm looking for a little help with the ESA1 form. I'm looking to claim for mental health issues of depression and paranoia which really have effected me since the death of my son.

The problem is im not good at describing things in detail and its really stressing me out that I may get this wrong and mess up my claim.

The parts of the form with is worrying me is;

Part 3 - "Please give brief details of your illness or
disability"

Part 22 - Other information "Please use this space to tell us anything else you think we might need to know."

If anyone could give me any advice and what type of things I need to write and details I need to give, it would be really appreciated.

The ESA1 is the basic claim form for ESA,

You should answer the question, as it is asked, give brief details of your illness, etc.

As for the other information section, you do not have to put anything here, unless you feel that it is absolutely necessary to your claim.

Once your claim has been processed, you should enter the Assessment Phase of ESA, you will then receive an ESA50 Questionnaire, which is a lot more in depth than the ESA1 form, you can see an ESA50 here, for future reference :

Hi C3,

You can put in a claim for DLA as well if you wish, if you think you can handle both assessment processes at the same time.

As long as you have suffered with the medical conditions and resulting limitations/disabilities/care needs for at least 3 months before your DLA claim, and expect to suffer with the same for at least 6 months after.
