 	 				ESA dilemma was created by chris o			
  			 				got the phone call from DWP a month ago saying that my ESA50 was on its way, two weeks later it hadn't arrive so i phoned to let them know.. they said they would send another form.. it didn't arrive either and i called again. they said it was posted on 19th jan and its still not here..
 
my sister completed the ESA50 PDF on her computer and im still waiting to see my doctor for medical reports before i post it.
 
what worries me is that i don't know where i stand with regard to the deadline date.. should i call them again or wait a bit longer & see what happens..?

I would contact them immediately to inform them that the second ESA50 has not been received. I suggest you confirm the address they are sending it to, even a minor mistake in house number or postcode could mean it is being delivered to the wrong address.
 
Having spoken to the DWP, I would follow up the call with a letter detailing the two missing ESA50s and your latest conversation.
 
Unfortunately, the DWPs default position is that if they've sent it, you have received it, so you need to put a stake in the ground now, to prevent problems later in your claim.

I would recommend you start completing this using the guides on the following link

So that you can return a copy as soon as you know where to send it (assuming the return date has lapsed).
