Need some help

Does anybody know where I can get a form for my go to fill in for my blue badge as I have been told my appeal for dla is going to be another eight months

Mine is my local Social Service office Louise. If it's the same for you they should be able to post a renewal form out to you if you ring and ask.
(want a laugh?......years ago, my local Blue Badge scheme was on the first floor of the local council offices and no lift....ah, the understanding of people without disablities!!)

Local Councils generally deal with the Blue Badge Scheme.

My local council sent a form to my GP themselves, I didn't need to have anything to do with that part.
