Hi G120,
As you did not have a face to face medical, the document that you should request from the DWP, would be an ESA85A.
This should include the prognosis, (recommended length of award), however it will probably not give a great deal of further detail, except for why you qualify for WRAG, and don't qualify for SG.
It's still worth requesting though, please see :
ESA medical – what forms to ask for? ESA Forms It may be wise to request all documentation used to assess your award of WRAG.
I do not believe that we have a template letter for this, however you could submit a SAR (subject access request) under Data Protection, however they will then have 40 days to comply.
You could request them over the phone, backed up with a recorded delivery letter.
If you are appealing, don't miss your 1 month deadline, if the ESA Forms have not arrived.
