DLA 80 Form was created by axle
Hi, Has anyone filled in a DLA80 form? At the back of the form is a section Statement from someone who knows you. Who is best to fill this in? My Consultant ,Doctor or Wife who is my carer?

The best person to complete this might be your partner, carer or someone else who knows you well and sees first hand the difficulties that you have with everyday activities.
Alternatively, it could be a professional involved in your care, such as your GP, specialist, occupational therapist or a nurse. In this case, make an appointment to see that person so you can answer any questions they might have and take the ‘Health professionals sheet’ from this guide with you.
Ask the person to complete the statement there and then if they can. If they can’t, or wish to write a longer report, ask them to send it to you and check when it will be done by, as you may have to send off the claim pack without it in order to meet the deadline. If this happens enclose a note explaining that you will send it as soon as you are able.

It's best to use the person who knows the most about the care and/or mobility needs you have.
