Report mentions Med 3

I'm currently going through my appeals bundle and note that they refer to my ESA50 and a Med 3 on the report. I am understanding that the Med 3 is a form of sick note from my doctor - do I have that right? This is strange as it's stated in the letter to my MP that my doctor didn't send in any form or letter and so the Atos/DWP decision was based on my ESA50. What Med 3 would they be referring to?
I had my last medical back in 2006 and the doctor deemed me unfit for work and my condition has changed dramatically since then and the DWP have been notified about that.

Hi TCP,
The Med 3 is the official name for what is now known as the "Fit Note", issued by your G.P., stating that you are unfit for work. (Formerly known as a "sick note")
This Med 3 may be some years old if you have not had one issued by your G.P. in recent times, and could be from around the time that you first became ill.

If this was issued as in my case 1999,from a GP in Watford, when I was more mobile then, & I suppose followed up periodically every time I saw a DWP Medical Consultant for DLA/IIB & again for this ESA in Oct 2011, whereby no medical examination took place. What are the deuce are ATOS/DWP up to??

When I had my medical Examination in 2011 I was on IB and had not sent any Medical Certificates in for over 12 years and the Dwp/Atos had not contacted my Doctor. I am thinking of writing to the DWP to ask for a copy of this MED 3.
