Which GP was created by misssarah89
On my ESA and my DLA form i remeber being asked to state the name of my GP.
I recently got my DLA declined saying they had used information from the form my GP filled in.
I am going to assume there is a form they send to you gp.
My question is.... Do they send it to the specific GP that you mentioned to fill it in or do they send it to the practice for any GP to fill in?

Generally they are addressed to the relevant GP at the practice however the letters may actually opened by the practice clerical staff.
However if the GP is away, sick or unavailable then it may not necessarily be the actual GP in question who completes it.
It maybe better to confirm with the respective Practice Manager to see what occurs if your own GP is not available for any reason and what the practice's policy on these matters is.

There is a form that the DLA Unit send to a GP for completion and if you specify a particular doctor, it should be addressed directly to him/her.
There is no guarantee however that when it is received at the practice it will be completed by the nominated GP. Many reports are completed by different GPs within a practice, by using evidence (or conversely the lack of it) from a patient's medical records.
If you discover that a medical report has been completed by a GP other than the one you have nominated who knows very little or nothing about your care/mobility needs, and it has subsequently had a bearing on the outcome of your DLA claim, this may form the basis of any potential appeal against the adverse decision.
