ESA3 ?

From comments elsewhere, an ESA3 is issued to ESA claimants who may qualify for ESA(IR) as a top up to their ESA(CB) due to low income.

A DS1500 is for claimants who are terminally ill and have a life expectancy of less than six months, if this is not relevent to you, then you should ignore it.

The ESA3 form covers a change of circumstances, usually with regard to claiming ESA(IR), either as a replacement for ESA(CB) or as a top-up, typically you need to complete the section related to your income, assets, and what your partner does.

If in doubt, phone your local DWP office to find out why it has been sent.
