An ESA3 form is used to capture details of your income and assets in relation to an Income Related benefit such as ESA(IR).
You might want to check why you have received the form, you may already be receiving ESA(IR), or you may be eligible for an ESA(IR) top-up to an ESA(CB) claim.
I can't say whether you need to complete the form, but if the DWP have requested the information, this usually implies they expect it to be completed and returned.
