Re the ESA forms see

ESA medical – what forms to ask for?

Re the DLA, it sounds like this doctor has been given the form to fill in, if this is the case, then I would suggest you see them, if only to ask if your normal GP can complete it for you.

The ESA Appeal guide explains how to complete the GL24. See

www.benefitsandwork.co.uk/help-for-claimants/esa

You can ask for a Paper hearing that you do not attend, or an Oral hearing that you do, the latter has a higher rate of success.

Hi SB,

As I am not sure now, whether your appeal is for ESA or DLA, :) see the following :

Employment and Support Allowance appeals
35 page guide to revisions and appeals for ESA.

Accessible from here :

ESA Guides

There is a template included of an initial appeal submission using GL24.

See also :

ESA FAQ’s

With respect to DLA :

Disability Living Allowance appeals.

Guides to help you through the entire appeals process.

Accessible from here :

www.benefitsandwork.co.uk/help-for-claimants

&

DLA FAQ’s

With respect to both DLA and ESA appeals, see :

Appeals FAQ’s

A claimant can request an oral hearing where they attend in person and give evidence, or a paper hearing where they do not have to attend.

Oral hearings have a better success rate than paper ones.
