I understand that you are supposed to inform DLA of any change in your medical conditions that may cause a change in limitations and care needs that give you the DLA award.
However, as you were already in receipt of the highest awards of Care and Mobility Components, and your conditions seem to have worsened, there would have been nothing to be gained , as this deterioration could not have increased your awards.
The DWP are more interested in an improvement or change in peoples conditions which may therefore decrease their limitations or care needs, as this could result in an overpayment.
Therefore, when it comes to filling in your ESA50, just tell it as it is, at the time of filling in the form.
