ESA3 Form.

Dear all I am going beyond myself at the moment- I have had 3 letters from JC+ informing me my CB ESA will end 30th April. I have requested an ESA3 form twice from jobcentre plus (to change to IR ESA), and neither have come. The local jobcentre doesn't hold these forms, and there are none on-line. I know there was a similar post by Pusscats mum, but surely there are many others out there in a similar position- is anyone else having trouble with this? Also, if no form comes, or doesn't come in time for me to transfer before 30th April, do I have to phone up on the 1st May and make a totally new claim for IR ESA? Seems crazy

I just replied to your answer to a post of mine. As you know, I had supposedly been sent out two ESA3 forms one on 30th March and again 4th this month which never arrived, yet I recieved 3 letters today all saying the same, benefit being stopped.
I was lucky that I got one yesterday from the jobcentre but they only had 2 left.
When I last phoned them on Tuesday they said they were not getting any more of these forms in as the form itself was being changed.
This is a ridiculous situation, leaving people without any money simply because they have no forms, yet are telling people 'it is in the post', However Gordon did put in my thread the possibility of getting permission to use the ESA1 form instead, seems it has the same questions as the esa3 and writing on it Replacement for the ESA3 form'

Thankyou Gordon and Mrs Hurtyback for your posts. I would be very grateful if it were possible to get a scanned form or any form of a form if you know what I mean!! Seems a stange thing to be desperate for, but I am!!

Seems you were blessed to get your forms Mrs Hurtyback- I do hope they process them in good time. As you say it does seem that they don't seem very readily available for such a time as this!
