STATEMENT OF FACTS was created by cw12
i have a doctors appointment tomorrow and am writing a statement of facts to update gp on my condition. could anyone advise me how detailed should the information be.i am still on income support and am awaiting the change to esa so i thought i better update gp on condition as he doesnt know me well.also whats best to say to him, just that id like to update him on my condition due to being unable to getting out much and lack of time with him, with thanks.

There's no hard and fast rule, but you do need to balance telling your GP about you are effected by you condition(s) and their replaying it word for word back to the DWP, which could be a problem, so less is better tham more.

In terms of content, use the ESA descriptors as a basis for your comments, the ESA Claim Guides may offer examples, hopefully, the GP will use similar terminology if asked by the DWP.
