DLA supersession, will form be the same? was created by lindat
I've searched faq but can't find anything. I am going to ask for a superssesion of my LRC and want to have as much as I can prepared before contacting dwp about it. Is it just the normal form you get to fill in or is there a different one for this? My occupational therapist is doing a report, and says she will highlight the deteriation in my health and the increase in my care needs since 2009 when I was first assessed and I am taking a blank copy of the form my gp filled in for my successful esa appeal for wrag to support group and asking him to do it for dla too.
my hubbie is going to go through your wonderful guides whilst completing the form and so thought I'd try and find out if the form would be the same.

I think its a different form number, but is substantially the same content and asks all of the same questions that the claim form does.

thank you Gordon my hubbie has to help me to the toilet and back 2-8 times a night so although my day care needs are mainly morning and evening I am positive I qualify for middle rate care, have done for nearly 2 years to be honest but scared to do anything as I have indefinate HRM and LRC. The stress of juggling debt is just too much so I have to do this.
