You should be sending your GL24 and any evidence to the same address, the DWP office that normally deals with your claim.

You don't need to send them together, there is in theory a window for you to send evidence to the DWP for it to be included in a Reconsideration. However, with basic processes seeming to have broken down in many offices, when that window actually is, is becoming increasingly difficult to find, so if you have your evidence available now, I would send it with the GL24, and if not, send it as soon after as possible.
