Unable to write to fill in DLA form what can I do?

I am trying to fill in the DLA form. Unfortunately I am disabled with MS and also sever arthritis in my hands. I cannot hold a pen to write for more than a minute or two before I both lose power and also experience severe pain. My writing is not particularly good but becomes illegible if I continue.

Is it acceptable instead of trying to write in the boxes to just type the answers on a separate sheet of paper, number the questions that they refer to etc and submit these stapled to the form with name and insurance number on all sheets? In the individual questions spaces I could then write 'see additional information'. My answers would not fit in the small boxes anyway and I would have to add further information at the end.

I am at a complete loss otherwise how to continue. I don't want to ask welfare rights as it would take many visits as I cannot do it all at once anyway.

Hi mommaduck,

If you do this, don't forget to make sure that your name and NINO are clearly marked on every sheet of paper, and that they are firmly attached to the form.

Could you manage the online PDF form where you can type in the boxes and print it out and you can also add extra pages if necessary too?

The form is a repeat form with part of it filled in by the DLA as they lost the first one so I would rather stick with this form and do the best I can.

Logic now says that if they accept a downloaded form then doing it the way I need to should be accepted.

I once went to the trouble of downloading a Pension Credit application form and filling it in and sending it off. Because I didn't phone them first and get them to send me the form, they denied ever receiving it hence I always send thing recorded delivery. At least you can prove that they got it even if they then go on to lose it :blink:

Yes, we usualy advise people to send things by recorded delivery, or at least get proof of posting, for that reason!

Most of my DLA forms have been completed by typing the questions which require longer answers, I then printed them out and used Blutak to stick them on the relevant pages. What I did was to type a doc out in Word, my NINO (National Insurance Number) and name were on every question then and I didn't have to type out all that info every time.

I prefer doing it this way as I can save my application/renewal in my documents file.
