Just received my limited capabilty work questionnaire. I am just wondering why the top left hand corner has a hole punch all the way through the form.

Absolutely no idea.

Perhaps it's part of the DWP's customer care programme and it is designed to make it easier for you to attach evidence to the ESA50.
