If you stated on your ESA50 that you need four weeks to arrange transport to any appointment, then in principle, you should be able to re-arrange the appointment without fault.
If you did not, then you should still be able to re-arrange the appointment, but you will not be able re-arrange it again, and if you fail to attend, it is likely that your file will be returned to the DWP.
