Hi there i received the fit for work form and i had filled it in and sent it back before i joined your site,i just answered all the questions as quickly as i could and gave it little consideration. I attended the medical and was awarded 0 points,only when i joined your site did i see the note saying if you are on the higher rate dla to phone the jobcentre and tell them that you should not have received one of these forms. Since i have already filled in the form and had the medical is it to late to do this. I would be very grateful for any information thank you

Which includes a guide on appealing an ESA decision.
If you feel that the decision to find you fit for work was wrong, you have one month to appeal the decision from the date of the notification letter.
You can do this by downloading and printing off the GL24 form from the link on this page :
How to submit an appeal
If you are to appeal, then you will also need a "fit note/sick note", (MED 3 ) from your G.P. to cover you from the date of the dissalowance of your benefit.
Once you have sent in the MED 3, and your GL24, you will then be entitled to Assessment Rate ESA.

Just to add, an award of any type, of DLA, does not exempt you from the ESA WCA
