ESA (GP's Report to support my claim) was created by suelynnw
Dear Sir/Madam,

My partner has been called into his GP's Office next week to see him because we have filled his 'changing over from IB to ESA questionnaire' in and they need supporting medical evidence. I am his registered carer and I explained his mental health illnesses are now managed by medication seeing as the health professionals can do no more for him. He also has physical illnesses too which I have also listed on the form as per your wonderful guides. I just wondered if you knew what questions we might encounter from his GP as this is worrying him immensley and it has worsened his conditions, could any one help me please??

Hi slw,

It is highly likely from what you have written that the G.P. has received an ESA113 to fill in.

You can view an ESA113, here :

It is important, if possible, that the G.P. reflects how the health conditions qualify your partner for either The WRAG or Support Group of ESA, whichever is applicable.

Rather that simply stating the medical conditions suffered, he should try and tie the resulting limitations in with the ESA Descriptors, see :

Qualifying for the WRAG

It is a positive sign that the G.P. has called you in to discuss this, as most people are totally unaware that their G.P. has received, filled in and returned an ESA113, until after the event.

Just to clarify things, this is your partners G.P. that has called him in, and not an ATOS appointment to have a face to face assessment?
