completing esa form for someone else

If you are completing it using the claimants own words, the you should use "I", if you are completing it using your words, because the claimant is not able to adequately express the problems they have, the use "they/he/she".hi, can anyone advise me on this querie please, when completing the esa50 form on behalf of someone else who can not do it themselves do i answer the questions for example : he can not manage to travel on public transport due to his mental health problems. or do i put i can not manage to travel on public transport due to my mental health problems, as though it is the claimant filling it in. i know i must complete the part about filling in the form for someone else but i am unsure if i should answer i or he. hope this makes sense as i am slightly worried as this is my first post. many thanks dave.
In both circumstances, you should explain why you have completed the form in preference to the claimant.

I would recommend that any comment is, if practical, is more detailed than this, for example; manual dexterity problems, Mental Health issues, etc.My carer completed my ESA50 for me and we decided to put at the top 'my carer is filling this form in for me as I can not longer do so', then carried on in the 'first person'.
