Hi
Received ESA form this am I am in shock.
I have a daughter who was disabled from birth and now 27 years old.
I have to fill in the form and I am literally having a nervous breakdown.
I have spent all day on the verge of tears and now come across this site.
I dont use gps for her in probably the last10 years, only accident and emergency.
GPs cant do anything for her and I know how to treat her and nurse her.
Can somebody offer help.
Do I need to pay to see a consultant?
One distressed mother/ carer/ nurse/ friend and companion for my daughter.
Help would be grateful to fill and understand what to do.

Many Thanks
Julie

Firstly donty panic. Remember your daughter h as a severe disability. Look at the guides on this site and take your time to fill up the form. You dont need a Consultants report but up to date medical evidence can be useful as well as any other evidence about her disability from others that know her well. in particular from social workers etc if involved

The guides give all the best advice and you can seek the face to face advice from a local advice agency

Hi jm, and : Welcome to Benefits and Work :)

Just in case your username is your real name, you may wish to change it, as the forum can be viewed by anybody, see : My full name is showing, how can I stop it?

As it seems that you have received the ESA50 Questionnaire, you may wish to consider : Using The PDF Version Of The ESA50 Questionnaire , whilst referring to the corresponding sections of our ESA Claims Guides for Physical and/or Mental Health Conditions, which are the 3rd and 4th links down this page :

ESA Guides

There is also other useful information on the above page, from the 1st and 2nd links.

If you are going to use the PDF ESA50, please see :

How long do I have to complete the ESA50?

ESA50 too big for the envelope

Sending Documents to the DWP or ATOS

Try not to get too distressed regarding this, although I do understand. :)

The most important issue is to explain how your daughter qualifies for an award of ESA with respect to the qualifying criteria, it is the limitations as a result of the health condition that are important, more so than the medical condition in itself.

These links may help :

Qualifying for the WRAG

Qualifying for the Support Group

You may also wish to review our : ESA FAQ’s

Please see this link for potential sources of professional advice :

Where to get advice?

Once you have had a chance to digest some of the above, please return to this topic if you have any other questions, and we will do our best to help as much as we can. :)
