Recieved esa form

Ive just received my esa form in a hand written brown envelope, and it contained only the esa form and an envelope with a p.o. box 34, bootle DBC, jupiter drive, chester address on it. Is this right? There was also no letter or date of return for it - there should be a letter right? who should I contact to query this?

I would contact ATOS to get one sent to you, don';t just accept a date over the phone, get it in writing.

Whilst the DWP request the ESA50 be issued, it is ATOS that actually does it, the date of return is set by the date that the form is sent.

Just one last question, is it normal for the form to returned to a p.o. box in chester?

I am inclined to wait a couple of weeks as this will give extra time to get all the info together and the supporting letters etc but I am also concerned that ATOS may claim that they have sent it out and it has got lost in the post.
