Does the fit note need to go with the ESA1 or do I wait to send it in with the ESA50? I'm not sure at which stage to send it in.

Ideally it needs to go with the ESA1, but I would not wait to send the ESA1 if you cannot get a Fit Note immediately. As soon as you have one, send it to the same address as the ESA1, remembering to take a copy of, with a short covering letter, see
