Claimants meeting descriptors but placed in WRAG

Since the forum reopened last week, I’ve read at least two posts by or on people of people who obviously meet LCWRA criteria as set out in ESA Reg 34 (1) and (2) on grounds of either physical or mental incapacity. In one recent post ESA Reg 25 (2) would probably be relevant. Despite this, the claimants concerned have been placed in the WRAG. No doubt, there are many examples in the archives of this forum and others. I know this is a huge question but to what can this issue be attributed?

"Inaccurate" assessments by the ATOS HCP, and/or poor quality decision making by the DWP ESA DM, to name just a couple of the possible reasons.
