DLA was created by SAN
Hi i am filling in my DLA form, My gp said that they would write him for any medical records, though im told they some times do not and it depends on what is put in form.. I was also told there is a form you can get sent to your gp to fill in before you send your claim of is this true and what is the form called.

I am not sure if this is what you are referring to but have a look at page 60 of the DLA Claims on Physical Grounds or page 52 of the DLA Mental Health guide.
