DLA fiasco - need letter help - please!

Organised?! My memory is so bad that I have to write things down all the time but then can't find the notes! I think it's more a case of using caution as I'm unfamiliar in this area and I do lack confidence these days but your replies are of enormous help to me. Thanks again.
