No 4 point PIP descriptors results

Possibility to score at least one 4-point daily living descriptor at the next review: Just had my assessment back only scored 2 on most so very unlikely to score 4 or more in any future assessment
Tasks you struggle with on a day-to-day basis: Getting a job, where are these jobs going to come from?
I struggle with finances when days are not going as planned in addition to dressing myself appropriately and making myself something to eat. I struggle with reality and have a false sense of reality. Hearing voices and having hallucinations. When I am well I am very much isolated apart from little help from the mental health team as the government have implemented cutbacks over the years and are in fact cutting further back still. It's very hard to trust other people so I tend to keep isolated. When unwell I am a hazard to both myself and others. I also suffer from disassociation whereby I don't know what I have done I can only refer to it like sleep walking or sleep talking yet being physically interacting with others unaware.
Expected income you will lose if the Green Paper cuts are imposed:

Support group of employment and support allowance I have recieved since I was 17 years old and personal independence payment before this I was awarded DLA indefinitly. I will lose throwing me into complete poverty It now seems the government are intent on stop supporting the most vulnerable in society. With the cost of living constantly increasing since the pandemic there is not much left over from both pip and esa support to languish a life on benefits.
Expected problems if you tried moving into work, with support from a work coach: Not being able to make the appointments and feel paranoid about who I'm talking with, fearful of mixing with others and afraid that if I become unwell of what I might do or behave.
Anything else to tell us: These welfare cuts have not been thought out very well and will impact a lot of people that will not be able to work due to the severity of their disability or illness. Even if people were be able to work where are the jobs going to come from when employment rates are already increasing.

After having 3 assessments 3 years periodically, and only one face to face why have the assessors not ever asked my GP for legitimacy or enquired to the local mental health services about my claim?

These assessments are a complete nightmare to have to face and it does not seem easy to claim as it is.
