PIP advice

One immediate comment. PIP like DLA and ESA is not awarded because of the condition but the limitations that result from it, so you really need to go through the PIP Claim guide to understand what the Descriptors for PIP test.

You have two options;

You can conform to the DWP requirement for recording facilities, this requires a twin recording deck that has been professionally tuned, you are probably looking at more than £1000

You can provide either a twin or two recording units, they can be cassette or CD but not digital. You may need to convince the DWP that they are capable of providing identical recordings.

In both cases you need to inform the DWP in advance and get their written permission to record, if you do not then the interview will not be allowed to start if you attempt to record it and if you persist you will be classed as failing to participate.

Or, you can covertly record the interview on a device of your choosing. However, you need to be aware that if the assessor becomes aware of your doing this they will terminate the interview and you will be classed as failing to participate. Whilst it is legal for an individual to covertly record a conversation for their own use there are legal restrictions that would prevent you distributing the contents to a third party.
