Freedom Pass Renewal

I am also a holder of a Freedom Pass (in a different borough from my friend)which expires in March 2015. I originally applied in 2008 and got a supporting letter from my GP. I am in receipt of DLA with a low rate mobility component and I am in the ESA support group. I am worried that instead of a straightforward renewal where an updated card is issued (as it was in 2010) I will like my friend be asked to fill in a new form and give fresh evidence for my eligability. My GP has since retired and new GP's are all locum's.
I have mislaid my original Freedom Pass application form and I am wondering if you can provide a guide to filling in a Freedom Pass form especially the section that asks about walking eg how far can I walk and how many minutes it takes. My main issues are mental ill health which amonst other symptoms causes me disabling fatigue and exhaustion and my energy levels are variable. Also can mental ill health be a sole criteria for eligability for a Freedom Pass?

I have contacted the Freedom Pass number but they are very unhelpful and would again kindly ask if you have any guides to help fill in a Freedom Pass renewal form. Or have any other members had to recently renew thier Disabled Persons Freedom Pass and what was the outcome and did they have to re-apply giving fresh medical evidence for eligibility?

I am afraid we do not have a guide in this area, you could have a look at the DLA Claim guides, things like the Freedom Pass and the Blue Badge are still based on DLA rather PIP, there is also a guide to "Giving Persuasive Information About How Far You Can Walk" in the same area.
