Thanks everyone for your advice. I would just like to say that it is great to have this forum and site for guidance and support.
Finally, last 2 points (I hope!) do you know if the PIP2 form can be completed electronically?
Also, I'm worried that it may be difficult to get the medical evidence within 4 weeks. Obviously, that should not be a problem with my GP but I would also need to see a consultant and I am also undergoing tests at the moment which could alter things.

Hi K10,
I am afraid that there is no Pdf interactive version of The PIP 2 Form as there is with The ESA50.
As you will see each PIP 2 Form has a Barcode on it, this Barcode ties the PIP 2 to you as an individual claimant.
Therefore even if you decide to use separate added A4 pages to provide details and evidence with regards to each applicable question, you would still have to use the original PIP 2 Form that you receive, even if it is to simply write by hand, See Attached Page xx of xx, in each applicable text box in the form.
You could then use your P.C. to type up the added A4 pages, which would save you having to hand write everything.
Make sure that you put your name, DOB, and National Insurance Number at the top of any added pages, it is also best to number each added page 1 of x, 2 of x, 3 of x, 4 of x etc, so that you can reference them easily within the PIP 2 Form.
Yes, it would be difficult to obtain an appointment with your Consultant within 4 weeks.
Options regarding this :
If you have the money, you could arrange a private consultation with your specialist to discuss your PIP.
You could contact your Consultant's secretary and ask them whether the Consultant would be prepared to provide a letter of support without seeing them.
You could add comments within The PIP 2 stating that you are awaiting a Consultation and will provide any fresh evidence as a result of this as soon as you have it.
Historical post Consultation letters and test result reports etc from your Specialists should be in your G.P. records.
For the future, when you attend any Hospital Outpatients Specialist Clinics you can make a verbal request that all future correspondence between your Consultant and G.P. are also sent to you.
There should be no forms to fill in and no charge for this.
