It will be Maximus as the will be the ones that will be making the decision on a home assessment.

I am thinking she could use the same letter only change the date and the address. I have not worked out what she needs to put as the address on the letter, whether it should be Maximus c/o DWP? And what address? DWP address or Maximus address?

You can send it with the form, but don;'t be surprised if they miss it.

1) I ve been reading bits about ESA and I see that there is a work-focused interview at 8 weeks into the claim. So would this be 8 weeks from when you make the very first phone call to claim for ESA ? I assume it would be and I assume at this point the ESA50 form would have been completed and sent off to DWP and prob with the assessors?
2)My understanding is that virtually everyone has to attend this first work-focused interview (because it is not decided whether you will be in the support or WRAG at this stage). Am I correct, if so what happens if like me you cannot attend a job centre due to health conditions for this first work-focused interview? Would they do it over the phone?

It is something you would have to discuss with the JC+ advisor, it is possible for them to hold the interview over the phone, to defer it or even to cancel it. They are required to take your limitations into account when requiring you to attend.
