SAR DWP address

To which address should I make the request both about my PIP and ESA. I would like to know if the new occupational therapist, has either done this herself, or contacted the DWP requesting I change from DLA to PIP, and if she is now waiting to get a copy of my PIP application.
Can I also request info in the SAR letter for anyone who contacted the DWP concerning me during this past year?

I don't know whether you can ask if anyone has contacted DWP about you but you can certainly ask for the SAT to include anything from/to a third party.
I don't see that DWP would be able to give anyone information about you or your claim. DWP could initiate a reassessment if they received a report that you were claiming fraudulently but they are more likely to book a compliance interview in these situations.

Or you can make the request in the form of a letter, as long as you make it clear that you are making a formal SAR under The Data Protection Act 1998.
Address the envelope : FAO The Data Controller, at the Benefits Office dealing with your PIP.
Write DPA 1998 SAR in front, top, left corner of envelope.
Once received, they have 40 calendar days to comply.
Sending Documents to the DWP.

They may have access to your medical information held at your doctors but the DWP would not be party to this.

Yes ESA and PIP are usually administered out of different offices.
