hi,i plan to apply to dla to get my condtion looked atagain.did you have to go for a medicaldid you get long form to fill in? and also did you have to fillin the pip form,i,m also worried about not having a photo copy of my first application eight yr ago,did you have yours hope you dnt mind me asking thanks
