Q13 says tell them how long it takes to complete this activity. Do they mean you put down all activities and times for each activity covered in going out.?
Section 2 I have a lot of health conditions and disabilities which I doubt I will not be able to remember them all, So just checking on this one. Are we allowed to take our own copy of the form to help us if we do need some help?
How do we do a diary for our everyday needs and do we have to cover every problem we have, every day, each day? Is there a template we can use and they will accept.
Sorry a few here, I have been extremely stressed going through the form and I just want to get it all right.
Probable be back with a couple more before my form goes in.
Thank you for your help it really has made the form a lot easier to understand.

Forgive me but I'm a bit confused. I can't find anywhere where Q13 asks about how long it takes to do this activity!

However, Q14 does talk about the time it takes you to complete a walk.
Yes you can take a copy of the form, if you need to refer to it then just explain why you are doing so.
There's a template for a diary in the PIP section. You need to try and write down everything that you do for at least a week and for a longer period if you can manage. Also, try and explain how those activities affect you.
So as an example, if your prepare and cook a meal but are exhausted afterwards and cannot do anything else then this is something else to include in the diary. If you have mental health issues and something you do during the day makes you feel down then again this is something to record as well.

Q13 does talk about the time of day that you are affected.
Thanks Gordon, I just checked it and it is in 13D Extra Information and says, tell us how long it takes for you to complete this activity.

Are you looking at a PIP2?
There is no 13(d) on the PIP2, in fact none of the questions have a (d) option.

I am very confused now. I have he PIP form to fill in the 40 page one which I thought was the PIP 2 form, not sure why and it has a Q13D which covers "Extra information - Going out? pulling mu hair out and anxiety went through the ceiling a couple of hours ago,
Could do with a break but have no time as my form has to be returned by the 3rd of November and I really do no want to ask for an extension as this may affect my DLA.
If you haven't seen a Q13d I will be happy to send it over to you so you can see it
Any help would be appreciated. Thank you.
