Medical factual report , what do i ask to put in

spoke to my gp this afternoon over phone, he is willing to do a medical factual report for me, he said for me to write on a4 the problems i have and how it affects he would word in his in way on a medical report , which is really good fantastic really , but the thing is i dont know how to to do this or how best to start , is there a template of sorts for medical report , as im guessing i cant ramble on for long on each topic , i got to take it into doctors tommorow morning so its a bit like homework any advice please cheers sharon

We usually suggest that the claimant prints out a list of descriptors, with the points missing, and let your GP comment about the things they think will apply. You do need to be careful not to put words into the GP's mouth as DWP can ask to see anything you have given your GP in writing to request the report.

Please could you clarify what you mean by putting together a list of descriptors, with the points missing as I'm not clear what you mean.

Not sure if you are a member of the site, but if you join or already have, you can download the full pips guide which has a list of the Descripters and HOW MANY POINTS YOU GET for each one. So you can also see then which descriptor you fit and even have an appt with the GP you spoke to and take some notes, he will then have a clearer idea and if he has seen you he is more likely to fit what you have told him in the particular descriptors this means your notes will put you in a specific descriptor.
DESCRIPTORS are IMPORTANT as they are the answers in the boxes and each carry how many points you get, so certain answers will have 0 points, if your answer i something you can do and does not apply to you.

Having this information before you visit your gp and take nets with you. Then ask him to destroy the notes when he has done it.

If you can get an appointment with your GP LOL but its worth trying, if the receptionist asks what your appt is about tell them its about ongoing conditions if they push for more info tell them its private.

Worth doing face to face but be armed with the information about points so you know where you are, then you can point him in the right area.

This is a GOOD OPPORTUNITY do cease it as lots of GPs are sick of the DWP and what they are doing and don't want to get involved.

You have a great chance here to get your claim accepted 1st time round.
