If she is using your words then she can use "I", if they are hers then she should use "he", in either case explain what you have done and why to avoid assumptions.

As far as I am aware there is only one version of the PIP2, but can I ask why you are looking at the sample PIP2, you must use the form that you have been sent as it is barcoded.
