Hi, my daughter has been sent an ESA50 to complete. She is currently in the WRAG group, but her mental health has worsened since she last completed a form - she has anxious personality disorder and GAD.
A few weeks ago she developed a fear of choking, after reading an article, and now has to constantly chew her food for ages before attempting to swallow it. I have had to encourage her to swallow her food at times, because she has been so fearful of choking on it.
We are unsure how to answer the tick box for this question:
Can you chew and swallow food and drink without help or prompting from another person?

She struggles with this at every meal, but as I work full-time, I am obviously not around to be able to prompt her. Can you advise please?

If she is affected the majority of the time then I would say that you have nothing to lose by completing this section, but you may well need medical evidence to support the contention that she meets the requirement.

Can I also please ask whether we always have to answer a tick box, or can we just write something to go alongside it? Will we be penalised for not ticking a box?

You must answer the question but if it is easy or more flexible for you to do this in words then that is fine, just mark the tick boxes as see below or wherever you are answering.
